Whom to target

  • The profile: GDC-registered dentist, 2–6 years post-VT, surgical appetite (confident extractions minimum; implant certificate a plus, not required). Hungry to become a full-arch surgeon — not an established implantologist (they'll demand 40%+).
  • Where they already are: your dental academy alumni (warmest pool you own), delegates of implant courses (ICE Dental, Full Arch Club, LDI cohorts), MSc implantology students, dentists posting cases in UK implant study clubs.
  • The insight: 21D hires exactly this profile at £85k starting, rising after mentored training — "attitude over certificates." The pathway is the product; the salary just needs to be fair.

🇪🇺 The European route — same skills, easier supply, lower salary pressure

Why it works: dentists with EEA-listed qualifications still get near-automatic GDC recognition — no ORE exam — confirmed extended to 2028. Bupa runs a whole EU/EEA recruitment pipeline on exactly this route. The window is open but time-limited: build the pipeline now.
  • Target first — EU dentists ALREADY in the UK (settled/pre-settled status): no visa, no sponsor licence, GDC-registered or fast to register. Thousands work in NHS corporates doing UDA dentistry and want out — your implant pathway is exactly the escape they're looking for.
  • Target second — implant-experienced dentists in Spain, Portugal, Greece, Italy, Romania, Poland: home earnings are commonly €25–60k; many already place implants at volume (implantology is strong in Spanish/Portuguese/Romanian private practice). A £70–90k salaried UK role + relocation support is a genuine step up — and they arrive at Tier 1–2, not Tier 0.
  • The catch for overseas hires — the visa: post-Brexit, EU citizens need a Skilled Worker visa. You need a sponsor licence (one-off, ~8 weeks to obtain — start now), then each hire needs a Certificate of Sponsorship. Irish citizens are exempt (Common Travel Area). Budget 3–6 months end-to-end for an overseas hire vs 14 days domestic.
  • English evidence: IELTS 7.0 / OET B (or qualification taught in English) for GDC registration — screen for this on the first call, it's the commonest delay.
  • Salary expectations [estimate]: UK-based EU dentists: £65–85k lands well (they're comparing against NHS associate income). Direct-from-Europe: £60–80k + relocation package (flights, first 3 months' accommodation, GDC + visa fees paid, ~£5–8k total) often beats a bare £95k offer. Slot them into the ladder at Tier 1 (£85k) or Tier 2 (£100k) based on the hands assessment — same gates as everyone.

The offer your ads sell

  • £85k–£120k salary + per-arch bonus after solo sign-off (e.g. £150–£250/arch above 8/month)
  • Guaranteed diary — marketing + TCO team fill the book; they never find a patient
  • 12-month mentored full-arch pathway with sign-off gates and pay rises attached
  • On-site lab — chairside technician, same-day conversion (your edge over 21D/Evo)
  • Zero risk — GDC + indemnity paid, private medical, CPD budget, birthday off

Become a Full-Arch Surgeon. Salaried. Trained. Booked.

Most dentists wait ten years and spend £30k+ on courses to place their first full arch. Our clinicians do it inside twelve months — mentored, salaried, and with the diary already full. Employed implant dentist, £85,000–£120,000 DOE, rising after our mentored programme, plus per-case bonus once signed off solo. You get: a structured pathway from guided single implants to immediate-load full-arch surgery; an on-site laboratory with chairside technicians; a treatment-coordinator team that fills your book; GDC and indemnity paid; private medical; CPD budget. You will NOT: find patients, chase lab work, run a business, or buy another weekend course that ends without a patient in the chair. Who we want: GDC-registered, 2+ years post-VT, surgical exposure (implant certificate a plus, not required). We hire for hands, judgement and hunger. Apply: CV + a paragraph on why full-arch. Coffee and a clinic tour first — no panel interviews.

12-month full-arch pathway — gated, pay rises attached to gates

Phase 1 · FoundationsMonths 0–2

Digital workflow (CBCT, intraoral scanning, guide design) · treatment planning · 20+ assisted single implants · occlusion & prosthodontics refresher · lab immersion week — they must understand what the technician needs.

GATE: plans 10 cases correctly · places supervised singles to protocol
Phase 2 · Mentored surgeryMonths 3–5

Solo single implants + immediate temporisation · extractions & socket management · bone-graft workflow (your £487 protocol) · assists on every full-arch case in the diary.

GATE: 20 solo implants ≥95% integration · competent immediate temps
Phase 3 · Full-arch apprenticeMonths 6–9

Second surgeon on arches: flap design, alveoplasty, implant distribution (4–6/arch), multi-unit abutments, conversion protocol with chairside technician · leads the prosthetic phase end-to-end · optional external immediate-load masterclass (~£5k) as an accelerant.

GATE: performs each stage solo under supervision across ≥10 arches
Phase 4 · Supervised soloMonths 10–12

Leads full-arch cases, mentor scrubbed in but hands-off · manages complications (failed integration, fracture, phonetics) · own patient list + TCO pairing.

GATE: 3 consecutive solo arches to standard → SIGN-OFF + PAY RISE
Phase 5 · ConsolidationMonth 12+

Solo at 8–15 arches/month · quarterly audit of integration & remake rates · begins mentoring hire #2 — each dentist trains their successor. That's how you scale without buying courses.

ONGOING: ≥95% integration · remakes <3%
Mentor decision: phases 3–4 need a lead full-arch surgeon — you, or a bought-in sessional mentor. If it's you, your chair time is the constraint for ~9 months. Settle this before the first ad runs.
Cost logic: external full-arch courses run £3k–£30k. Internal mentorship on your own live cases costs a few slower cases in months 3–9 — far cheaper, and exactly how 21D does it.

Hiring protocol — 14 days application to offer

  1. Screen (48h): CV + one-paragraph motivation. Filter: GDC, 2+ yrs post-VT, surgical appetite. Attitude over certificates.
  2. Call (20 min, week 1): you or lead clinician. Sell the pathway; probe hunger — "where do you want to be in 3 years?"
  3. Clinic day (week 2): half-day shadowing a live arch + meet the lab + lunch. This is the close.
  4. Hands assessment: bench test — suturing, model placement, plan a CBCT together. Judgement, not perfection.
  5. Offer within 5 days: written — salary, gated pathway with rises attached, bonus, benefits, 6-month review.
  6. Contract: employed (17% on-cost budgeted), 3–6 mo notice post-training, training-cost clawback (24-mo taper), non-solicitation. Draft it properly.

Their week — 2 surgery days + 3 clinic days

TimeSurgery day (×2/wk)Clinic day (×3/wk)

08:30–13:00Full-arch surgery: extractions, placement, immediate conversion with chairside technicianConsults & CBCT planning (3–4 new patients with TCO) · single implant placements
14:00–17:00Fit & occlusion · post-op · second arch or reviewsReviews · records for final bridges · try-ins · implant crowns
17:00Mentor debrief · log case in audit registerLab handoff · pipeline review with TCO
  • Case audit ownership — integration/remake register, presented monthly (governance + learning loop)
  • Content capture — consented before/afters every arch day; feeds the marketing that fills their own diary
  • TCO conversion review — weekly; the dentist's follow-up call closes hesitant full-arch patients
  • Lab liaison — weekly 30 min with technicians on design, materials, remake causes
  • Mentoring hire #2 (from month 12) · Academy teaching (optional, paid) — closes the recruitment flywheel

30-day launch checklist — what to do now

Week 1: decide the mentor (you vs sessional) · fix salary band & bonus · draft contract with clawback
Week 1: email the academy alumni list with the WhatsApp copy · announce £2k referral bounty to staff
Week 2: post Indeed (sponsored) + BDJ Jobs · publish careers page with the long-form ad
Week 2: LinkedIn: organic post + 20 direct approaches to implant-course completers
Week 3: film one full-arch day → cut the 30s reel → £500 test spend targeting UK dentists
Week 3–4: run screens + calls as they come · book clinic days · bench-test kit ready
Week 4: first offer out · tracker tab live from their month 1 · review channel performance, double down on what produced candidates

The clinical ladder — pay bands, permissions, and what each band must earn

Loaded cost = salary + 17% on-cost (employer NI 15% above £5k + 3% pension — verified vs 2026/27 rates). Break-even uses current calculator economics (arch contributes ~£6,967, implant ~£1,487 incl. streams). Every promotion is a gate passed, never a tenure reward.
Tier 0 · Foundation Dentist — £50,000loaded £4,875/mo

CAN: new-patient consults & treatment planning support, restorative dentistry, hygiene-phase care, suture removal & post-op reviews, assists on every implant & arch case, CBCT records, denture/temp maintenance. In the training pathway: Phase 1 (foundations).

CANNOT: place any implant, administer surgical phases, consent surgical patients alone, touch a full arch except as assistant.

MUST DELIVER: covered by general dentistry income + freeing ~30 senior chair-hours/mo (worth ~£15k of arch capacity). Their real value is releasing your surgeons.

PROMOTION GATE: Phase 1 sign-off — 10 correct case plans + supervised suturing/soft-tissue competence → Tier 1
Tier 1 · Implant Dentist (Mentored) — £85,000loaded £8,288/mo

CAN: single implant placement under mentorship → solo as competence proves, bone grafts (your £487 workflow), immediate single temps, implant crowns, second-assistant on arches, own review list. Pathway Phases 2–3 entry.

CANNOT: lead any full-arch surgery, immediate-load an arch, manage surgical complications alone, work unmentored on grafted sites until signed.

MUST DELIVER: break-even at ~5.5 implants/mo; target 12–15 implants/mo (≈£18–22k contribution) + assisting on all arch days.

PROMOTION GATE: 20 solo implants ≥95% integration + competent immediate temps → Tier 2
Tier 2 · Advanced Implant Dentist — £100,000loaded £9,750/mo

CAN: solo singles & grafts at volume, second-surgeon on every arch (flaps, alveoplasty, distribution, MUAs), leads the arch prosthetic phase end-to-end (records, try-in, fit, occlusion), conversion protocol with chairside technician, supervised solo arches late in tier. Pathway Phases 3–4.

CANNOT: solo immediate-load arches without mentor scrubbed in, take sole consent for full-mouth cases, manage arch complications unsupervised.

MUST DELIVER: break-even ~6.4 implants (or 1.4 arches); target 8–10 implants + 2–4 supervised arches/mo (≈£26–35k contribution).

PROMOTION GATE: 3 consecutive solo arches to standard, mentor hands-off → Tier 3 + pay rise
Tier 3 · Full-Arch Surgeon — £120,000–£150,000loaded £11,700–£14,625/mo

CAN: solo immediate-load full arches (All-on-4/6), full surgical consent, complication management (failed integration, fractures, phonetics), own arch list with TCO pairing, per-arch bonus active (£150–250/arch above 8/mo). Pathway Phase 5.

CANNOT: zygomatic/remote-anchorage cases (external referral or further training), train juniors solo until 6 months at tier.

MUST DELIVER: break-even 1.7–2.1 arches; target 8–12 arches/mo (≈£56–84k contribution) + overflow singles.

PROMOTION GATE: 6 months at ≥8 arches/mo, audit ≥95% integration, remakes <3% → Tier 4
Tier 4 · Lead Surgeon / Mentor — £150,000+ (+ bonus)loaded £14,625+/mo

CAN: everything clinical, plus: mentors Tiers 0–2 (this is how you scale without buying courses), signs off promotion gates, owns the clinical-governance audit, teaches in the academy (paid), complex/revision cases.

CANNOT: be the only person who can do arches — their explicit job is to make themselves replaceable.

MUST DELIVER: 10–15 arches/mo personally + one Tier 1–2 dentist progressing a phase per quarter.

This is the mentor role Phase 3–4 of the training pathway depends on. Hire or become this first.
How to use the ladder: advertise Tier 1 (£85k — matches 21D's entry) and Tier 0 (£50k) from your academy pool. Never pay a tier's salary without enforcing its gate — the gates are what make the pay defensible, the indemnity clean, and the promotion feel earned rather than negotiated.
The economics of the ladder: every tier break-even is tiny against target (a £150k surgeon is covered by 2.1 arches and targets 10+). The ladder's real function is supply: you manufacture your own full-arch surgeons in ~24 months from a £50k start, instead of bidding for finished ones at 40–45%.
